Interprofessional Primary Care Teams
Information on the call for proposals to expand our primary care capacity in specific North York neighbourhoods.
Earlier this year, the Ministry announced new funding for primary care teams to provide attachment for people without a primary care provider. Five postal codes within NYTHP OHT’s catchment are included in the first round (see table below). We reached out to all PCN members to offer our support.
