North York Community Care Clinic
Our clinic provides ongoing health care for patients who do not have access to a family doctor or nurse practitioner on a regular basis.
What is the North York Community Care Clinic?
Please note that the last day of operations for the NYCCC is March 31, 2026.
We recommend any patients in need of attachment to a primary care provider to contact our North York Primary Care Community Connect team by completing this online form or reach out to the Get Well Family Health Team (see details below).
For those who need to be seen more urgently by a primary care provider, please refer them to the North York Primary Care Access Clinic by calling 416-639-9572.
The North York Community Care Clinic (NYCCC) can connect patients with a nurse practitioner on a temporary basis to ensure they receive necessary care while they wait to be connected to a permanent primary care provider (family doctor or nurse practitioner).
This clinic provides essential health care services, including health checks, chronic condition management (for diseases such as diabetes or heart disease), prescriptions, cancer screenings and vaccinations. Patients will receive care through our partner, Baycrest Hospital.
Our clinic is open Monday to Thursday from 4:00pm to 8:00pm.
Eligibility criteria
To access the North York Community Care Clinic:

You cannot have an existing family doctor or nurse practitioner.

You must live close to the clinic below and be able to attend in-person:

If you know of any people that are currently looking to be permanently connected to a primary care provider and fulfill the following criteria:

Have valid health coverage - either OHIP (Ontario Health Insurance Plan) or IFHP (Interim Federal Health Program) coverage.

Do not already have a primary care provider (family doctor or nurse practitioner).

Live in North York (priority will be given to those who live in the M2N or M3H postal code areas.

The North York Community Care Clinic is a collaboration between North York Toronto Health Partners Ontario Health Team (NYTHP OHT), Get Well Clinic, Baycrest Hospital and Flemingdon Health Centre, and is funded by the Government of Ontario.
