We provide a forum for communication, dissemination of information, and represent the interests of the primary care community within the North York Toronto Health Partners (NYTHP) Ontario Health Team (OHT).
Joining the PCN is completely voluntary and a great opportunity for primary care
clinicians in our community to come together as a group to help shape an
integrated health care system.
