Join our Primary Care Network (PCN) to help co-design the OHT, connect with community providers and learn about new programs to support your patients and practice in the North York community.

 

Membership is open to primary care providers, allied health professionals and medical administrators in North York.

By joining the PCN, primary care providers will:

  • Have a collective voice that is unified, strong and effective;

  • Benefit from improved connections between the primary care sector and specialists, home care services and other community providers to improve patient care and primary care provider experience;

  • Lead access to integrated clinical and digital solutions for primary care providers and co-design integrated care.
