Interprofessional Primary Care Teams
Information on the call for proposals to expand our primary care capacity in specific North York neighbourhoods.
The Ministry has announced new funding for primary care teams to provide attachment for people without a primary care provider. Five postal codes within NYTHP OHT’s catchment are included in the first round (see table below). We’re reaching out to all PCN members to offer our support.

We are requesting that any NYTHP primary care providers or organizations that are considering submitting a proposal to please complete the PCAT Information Template by 5:00 p.m. on Wednesday, April 16th, to support planning and collaboration.
Next steps

Up to five proposals will be submitted, with one per assigned postal code (see table above) and all submissions are due by 5:00 p.m. on Friday, May 2, 2025 so we are working with an extremely compressed timeline, and we thank you in advance for your forbearance with this process.
