NYTHP is built on the foundation of strong primary care physician leadership as it understands that primary care providers have tremendous insight into the challenges facing our system and, therefore, have unique perspectives on how to address them.
Our NYTHP Primary Care Network (PCN) was established in April 2021 to represent the voice of primary care in North York. The Primary Care Advisory Council (PCAC) is composed of 11 primary care providers and a patient and caregiver health council member to represent the broader primary care network of providers in NYTHP decision-making and service planning. PCAC members sit at numerous OHT committees to provide the primary care voice in OHT planning.
